Your information, and what we do with it.

This notice explains what we record about you, why we record it, how long we keep it, and what you can ask us to do with it. It covers both the clinic and this website.

Who we are

Failov Medical Services is an ophthalmology practice operating from Platinum Embassy Hospital, No 3, Dr Rasaq Akindele Street, opposite new Ede City Stadium. We are the data controller for the information described in this notice, which means we decide how and why it is used and we are responsible for keeping it safe.

What we collect

For patients, we hold the information needed to care for you properly and safely:

  • Your name, age, contact details and, where relevant, next of kin.
  • Your medical history, including conditions, medication and previous eye care.
  • Clinical records created during your visits — examination findings, measurements, images and scans, diagnoses, prescriptions, treatment and surgical notes.
  • Correspondence about your care, including referrals to and from other practitioners.

If you contact us through this website, we receive only what you type into the appointment form — your name, phone number, an email address if you choose to give one, and your message. That is sent to the clinic by email and is not stored on the website.

Why we use it

Your clinical information is used to examine, diagnose and treat you, to keep a continuous record so that anyone caring for you knows your history, to arrange follow-up appointments, and to meet the professional and legal obligations that apply to a medical practice. Your contact details are used to reach you about your care — to confirm an appointment, to give a result, or to follow something up.

We do not use your information for marketing, and we do not sell it to anyone.

Our lawful basis

Under the Nigeria Data Protection Act 2023, we rely on your consent to examine and treat you, on our legitimate interest in running a safe and properly documented practice, and on our legal obligations as a registered medical practice. Health information carries additional protection under the Act, and we treat it accordingly.

Who we share it with

Your information is confidential. We share it only where your care or the law requires it:

  • Other clinicians involved in your treatment, including any specialist or hospital we refer you to.
  • Laboratories and diagnostic services acting on our instruction.
  • Your insurer or employer where you have asked us to make a claim or provide a report on your behalf.
  • Public authorities, where we are legally required to disclose information.

We will not discuss your care with a family member or anyone else without your agreement, except where you are unable to give it and it is in your interest that we do.

How long we keep it

Clinical records are kept for as long as is necessary for your continuing care and to meet our professional record-keeping obligations. Enquiries that do not lead to an appointment are kept only as long as needed to respond to them.

How we keep it safe

The practice runs on secure electronic records rather than paper files, which means access is restricted to the clinicians and staff who need it, changes are traceable, and records cannot be mislaid or read by whoever happens to be passing. Everyone working here is bound by a duty of confidentiality.

This website

The website sets no analytics or advertising cookies and does not track you. Two things on the site are provided by others and operate under their own terms: the map on the contact page is embedded from Google Maps, and the WhatsApp links open a conversation in WhatsApp. Using either of those means interacting with those companies as well as with us.

Your rights

Under the Nigeria Data Protection Act 2023 you may ask us to:

  • Give you a copy of the information we hold about you.
  • Correct anything that is wrong or incomplete.
  • Explain how a particular piece of information is being used, and who has seen it.
  • Restrict or object to a particular use of your information.
  • Withdraw consent you have previously given, where our use of the information depends on that consent.

There are limits: we cannot delete a clinical record we are obliged to keep, and we may need to confirm your identity before releasing anything. We will respond to any request within the period allowed by the Act.

We would rather hear from you directly, but you also have the right to complain to the Nigeria Data Protection Commission.

Changes to this notice

If we change how we handle your information, we will update this page and change the date shown above.
